Job summary

Masonicare Corp in Wallingford, CT is seeking an Assistant Clinical Manager (Home Health) to oversee planning, organization and delivery of high-quality home health care for adult and geriatric clients. You will supervise nursing staff, coordinate referrals, schedules and ensure compliance with policies and regulations.

The role requires a BSN or ANA community health nurse certification and at least two years of RN Case Manager experience, with CT RN license.

Qualifications

• BSN degree or ANA community health nurse certification or equivalent coursework in community health nursing or health care management.
• Minimum of two years’ experience as an RN Case Manager (home health).</br>
• Current Connecticut RN License.

Responsibilities

• Assigns qualified staff to meet needs of adult/geriatric clients and reviews work.
• Supervises nursing staff in delivery of services.
• Prepares on-call schedules and daily assignments; facilitates staff huddles and meetings.
• Reviews patient records to assess care plan and utilization of services.
• Mentors staff; participates in orientation and ongoing development.
• Participates in staff recruitment, selection and discipline with HR.
• Performs home visits to mentor staff and evaluate care quality.
• Maintains OASIS data and orders for service and reports.
• Ensures compliance with MIS data updates and documentation.
• Communicates with clients respecting age, culture, and needs.
• Completes annual education and medical requirements.
• Manages patient caseload with MD-ordered visits.
• Performs other duties as required.
