Qualifications

• Requires BSN or higher with nursing management coursework or equivalent.
• Current Connecticut RN license is required.
• Experience as an RN Case Manager in home care is preferred.

Responsabilités

• Oversee delivery of nursing care to adult and geriatric patients in home health.
• Supervise nursing staff, assign work and review performance.
• Coordinate on-call schedules and daily patient referrals.
• Ensure accurate clinical documentation and care plans are in compliance with policies.
• Support staff development, onboarding and mentoring activities.
