Job summary

Addus HomeCare is seeking a Home Care Service Coordinator/Case Manager for the Missoula area. You will schedule and supervise in-home care workers and clients, ensuring the care plan is followed and services are uninterrupted.

The role requires strong interpersonal and organizational skills, reliability, and proficiency with Word, Excel, and scheduling software. Hours are typically Monday to Friday, 8 AM to 5 PM, with potential travel to nearby areas.

Qualifications

• Interpersonal, organizational and communication skills are essential.
• Proven reliability and ability to manage schedules.
• Experience in home care or related field preferred but not required.
• Computer skills including Word, Excel, and scheduling software required.

Responsibilities

• Schedules and supervises in-home care workers and clients in a geographic area.
• Provides supervision and case management to ensure care plans are followed.
• Coordinates coverage and updates to ensure client services are uninterrupted.
• Maintains confidentiality and adheres to regulations and policies.
