Job summary

A&C Private HomeCare is seeking a compassionate Care Coordinator to join our in-office team. This role coordinates caregiver schedules, supports clients and caregivers, and ensures top-quality customer service while upholding agency policies.

Qualifications

• Knowledge of home care regulations and compliance requirements is preferred.
• Familiarity with state regulations, Medicaid waiver programs, and VA requirements is highly desirable.
• Understanding of caregiver credentialing, documentation standards, and regulatory compliance.
• Ability to maintain confidentiality and ensure compliance with HIPAA and agency policies.

Responsibilities

• Coordinate and maintain caregiver schedules.
• Match caregivers with clients based on client needs, availability, and caregiver qualifications.
• Communicate with clients, caregivers, family members, hospitals, and case managers.
• Complete caregiver onboarding and orientation.
• Assist with recruitment, interviewing, and hiring of caregivers.
• Monitor caregiver compliance, credentials, and required documentation.
• Conduct routine follow-up calls with clients and caregivers to ensure satisfaction.
• Maintain accurate documentation and electronic client records.
• Assist with call-outs, schedule changes, and emergency staffing needs.
• Ensure agency policies, procedures, and state regulations are followed.
• Work closely with the management team to promote quality care and excellent customer service.
• Participate in team meetings, training, and quality improvement initiatives.
