Job summary

Care Advantage Inc. in Charlottesville, VA is seeking a dynamic Registered Nurse (Home Care Supervisor) to lead our Personal Care Team on a full-time basis. This role focuses on clinical leadership, oversight of caregivers, and ensuring high-quality home-based patient care within our community.

You will mentor staff, coordinate individualized care plans, and supervise the delivery of services to patients and families, while upholding company values and regulatory standards.

Qualifications

• Active RN license in Virginia (compact/multi-state accepted).
• Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable).
• Associate's Degree in Nursing or higher.
• Strong communication and presentation skills.
• Ability to work collaboratively across teams.
• Reliable transportation for home visits (mileage reimbursed).
• Comfortable with technology (computer/iPad documentation).
• Willingness to travel regularly within assigned area.

Responsibilities

• Meet new patients, explain services, and complete start-of-care assessments.
• Develop tailored plans of care addressing medical needs and family involvement.
• Monitor patient progress and report changes to physicians and care teams.
• Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details and expectations.
• Conduct supervisory visits to ensure compliance and quality care.
• Lead or oversee training sessions for new staff on safety protocols and clinical standards.
