Job summary
Care Advantage Inc. is seeking a dynamic Registered Nurse (RN) Home Care Supervisor to provide clinical leadership across Staunton and Charlottesville areas.
The role oversees care delivery, mentors caregivers, and ensures regulatory compliance with PRN scheduling and local travel. You will lead start-of-care assessments, develop individualized plans, monitor outcomes, and coach nursing staff to maintain exceptional home-based care for patients and families.
Qualifications
- Active RN license in VA (Compact License accepted where applicable).
- Associate Degree in Nursing (ADN) or higher.
- Minimum of one year of nursing experience.
- Strong leadership, communication and interpersonal skills.
- Willingness to travel throughout the service territory.
- Proficiency with electronic documentation and technology.
Responsibilities
- Lead clinical supervision and oversight for home care teams.
- Conduct comprehensive patient assessments and admissions.
- Develop and oversee personalized plans of care.
- Monitor patient progress and outcomes; ensure quality and compliance.
- Coach caregivers and nursing staff; reinforce best practices.
- Maintain accurate nursing documentation and regulatory compliance.
