Job summary

Care Advantage Inc is seeking a Registered Nurse (RN) Home Care Supervisor to provide clinical leadership and oversight across Virginia Beach VA and surrounding areas. This role focuses on patient care, caregiver support, and program development.

You will conduct assessments, develop care plans, supervise in-home visits, and ensure regulatory compliance while collaborating with physicians and families.

Qualifications

• Active RN license in MD and/or DC (Compact License accepted).
• ADN or higher nursing degree.
• Minimum of one year of nursing experience.
• Strong leadership and interpersonal skills.
• Willingness to travel throughout the service territory.
• Proficiency with electronic documentation and technology.

Responsibilities

• Conduct comprehensive patient assessments and admissions.
• Develop personalized plans of care in collaboration with physicians and families.
• Monitor patient progress and clinical outcomes.
• Educate and support family members participating in patient care.
• Coach caregivers and nursing staff to follow best practices.
