Job summary

Community Health Network is seeking an experienced Home Health RN Case Manager to support patients in the Beverly, Marblehead, Salem and Swampscott, MA area. This full-time role offers 40 hours per week, with potential weekend coverage and 13-week contracts.

You will manage a caseload from admission to discharge, supervise LPNs and Home Health Aides, and require at least 1 year of home care RN case management with OASIS visits for a Medicare certified agency.

Qualifications

• Minimum 1 year of Home Care RN Case Management experience.
• Experience performing OASIS visits for a Medicare certified agency.
• Strong ability to manage a caseload and supervise staff.

Responsibilities

• Manage a patient caseload from admission to discharge in the community.
• Supervise LPNs and Home Health Aides to ensure quality care.
• Coordinate care across disciplines and agencies as needed.
