Community Health Network, Inc. seeks a Home Care RN Case Manager to serve patients in the Amherst/Hadley/Hatfield/Williamsburg, MA area. The role is full-time (40 hours/week) with occasional weekend rotation and mileage reimbursement.

Requires at least 1 year of Home Care RN Case Management experience doing OASIS visits for a Medicare certified agency. The RN will manage a caseload in the community, supervising LPNs and Home Health Aides, and ensuring smooth admission through discharge.

Qualifications

• Requires minimum 1 year of recent Home Care RN Case Management experience.
