Job summary

Granite VNA in the Greater Concord Area seeks a Hospice Community Registered Nurse (RN) to deliver in-home and facility-based care under NH Board of Nursing guidelines. This full-time role reports to the Director and involves assessment, planning, intervention, and collaboration with the care team.

Qualifications

• Graduate of an accredited school of nursing and NH nursing license (or compact state license).
• Three to four years of recent general medical/surgical or skilled/acute care nursing experience.

Responsibilities

• Develops and implements appropriate plan of care (POC) in partnership with patient, healthcare provider, and caregiver.
• Documents accurate ongoing assessment, treatment and discharge information in accordance with agency guidelines.
• Communicates with physicians, Hospice team, and care conference participants regarding current POC.
