Resource 1 Homecare Staffing in Concord, NH is seeking a Home Health & Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in patients' homes and communities.

This role offers a 50/50 split between Home Health and Hospice, flexible full-time scheduling, training for transition to home-based care, mileage reimbursement, and comprehensive benefits, including insurance, retirement, PTO, and holidays.

Qualifications

• Active RN license in New Hampshire.
• Minimum of 1 year of RN experience.
• Home Health and/or Hospice experience preferred.
• New graduates are not eligible.
• Valid driver's license and reliable transportation.
• Strong assessment, communication, and documentation skills.
• KanTime experience is a plus but not required.

Responsibilities

• Provide skilled nursing care to Home Health and Hospice patients in their homes.
• Complete comprehensive nursing assessments and develop individualized care plans.
• Manage an average of 5 Home Health visits or 4 Hospice visits per day.
• Monitor patient progress and adjust care plans as needed.
• Provide pain and symptom management for Hospice patients.
• Educate patients and caregivers on disease management, medications, and treatment plans.
• Coordinate care with physicians, therapists, social workers, chaplains, and other members of the interdisciplinary team.
• Document patient care accurately using KanTime EMR.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
