Job summary

Granite VNA is seeking a Hospice RN to provide compassionate in-home hospice visits across New Hampshire. You will build trusting relationships, perform comprehensive assessments, and develop individualized care plans in collaboration with physicians, social workers, and the care team.

You will deliver symptom management with a focus on comfort and quality of life, document interventions per agency standards, and educate patients and families.

Qualifications

• Graduate of an accredited school of nursing.
• Current New Hampshire nursing license or compact state license through the Nurse Licensure Compact (NLC).
• Three to four years of recent general medical/surgical or skilled/acute care nursing experience.

Responsibilities

• Build meaningful relationships with patients and families while explaining hospice philosophy and available services.
• Complete comprehensive patient assessments and develop individualized plans of care with the team.
• Deliver skilled nursing care focused on comfort, symptom management, and quality of life.
• Document assessments, interventions, and care plans per agency standards.
• Provide education to empower patients and caregivers.
• Collaborate with physicians, social workers, chaplains, and other professionals for coordinated care.
• Participate in team meetings, quality improvement, and ongoing education.
• Travel to patient homes in Granite VNA service area using personal vehicle.

Education
