  1. Build meaningful one-on-one relationships with patients.
  2. Provide individualized, compassionate care.
  3. See the direct impact of your work every day.
  4. Focus on one patient at a time rather than caring for multiple patients simultaneously.
  5. Help patients achieve their goals in the comfort and familiarity of their own homes.
  6. Enjoy autonomy, flexibility, and the satisfaction of making a meaningful difference in your community.

Whether you're traveling through vibrant downtown communities or scenic New Hampshire back roads, you'll have the opportunity to provide care where it matters most-at home.

  • Intervention
  • Implementation

The RN practices within the scope of practice defined by the New Hampshire Board of Nursing and works collaboratively with patients, caregivers, healthcare providers, and other members of the healthcare team.

The RN is also responsible for supervising Licensed Practical Nurses (LPNs) and Licensed Nursing Assistants (LNAs) assigned to patients, in accordance with applicable federal and state regulations.

Essential Functions

  • 7. Coordinate the patient's plan of care with members of the Interdisciplinary Team (IDT) and other healthcare professionals.
  • 8. Participate in team and patient care conferences and update short- and long-term goals based on interdisciplinary assessments and reassessments.

  • 4. Maintain professional nursing standards in all interactions with patients, caregivers, healthcare providers, and members of the healthcare team.
