Home Care RN Position Purpose:

Provides primary nursing care to home-based care patients as ordered by the physician, and in accordance with the organization’s policies and procedures. The RN is responsible for initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support and supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management, and aims for top decile client outcomes and patient satisfaction.

What You Will Do:

• Assess patient condition and risk accurately at admission, at required time points, and as needed, using appropriate documentation.
• Determine the appropriate level of care based on the patient's needs and condition.
• Develop an assessment-based plan of care in collaboration with the physician, Population Health Case Manager, and patient, including attainable goals and needed services to improve or stabilize the patient's condition and promote independence.
• Maintain current knowledge of applicable laws, regulations, policies, and ethical standards, and follow the Code of Conduct.
• Report changes in patient condition promptly to the physician and/or Population Health Case Manager, and obtain orders for any necessary changes in the care plan.
• Follow treatment orders and provide services as indicated in the plan of care.
• Use and implement appropriate educational tools and pathways as per guidelines and policies.

Minimum Qualifications:

• Graduate of an approved nursing program; BSN preferred.
• Current RN licensure in Connecticut.
• At least one year of experience as an acute care nurse.
• Home care experience is preferred but not required.
• Valid driver’s license and reliable transportation are required.
