Job summary

Trinity Health Of New England At Home in West Hartford is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes, develop care plans, administer medications, and educate families to support recovery. The role emphasizes 1:1 patient relationships with autonomy and professional growth.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in Connecticut.
• One year of acute care nursing experience.
• Home health experience preferred but not required.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Create individualized care plans and administer medications.
• Educate patients and families to support recovery and independence.
• Provide 1:1 patient care with autonomy and professional judgment.

Location: West Hartford

Status: Mon – Fri 8 am to 5 pm, weekend rotation every 4 weeks, 2 holidays

REQUIREMENTS:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation
