Trinity Health Of New England At Home is seeking a Home Care RN to deliver primary nursing care in patients' homes across the Hartford Region. You will assess, plan, and implement care in collaboration with physicians, supervise aides and LPNs as needed, and support patient outcomes and satisfaction with compassionate, mission-driven care.

Schedule: Monday–Friday, 8 am–5 pm, with one weekend every 4 weeks and two rotating holidays; no on-call. A $10,000 sign-on bonus is paid over two years.

Qualifikationen

• Graduate of an approved nursing education program.
• Licensed Registered Nurse in Connecticut.
• 1 year acute care nursing experience preferred.
• Valid driver's license and reliable transportation.

Position Purpose

Our Home Care RN’s provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. They also provide functional support/supervision to Home Healthcare Aides and LPNs as needed and therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.

Position Details

Bonus: $10,000 sign-on bonus paid out over two years.

Minimum Qualifications

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in the State of Connecticut
• One (1) year experience as a professional acute care nurse
• Home health care experience preferred but not required
• Ability to work with minimal supervision and maintain professional relationships with clients and families
• Valid driver’s license and reliable transportation required
