Job summary

Trinity Health Of New England At Home in Hartford region is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes, develop individualized care plans and educate families.

This full-time role offers a 4-week weekend rotation, opportunity to practice at top of license with autonomy, and a $10,000 sign-on bonus with competitive benefits.

Qualifications

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence in home health care.
• Practice at the top of your license with autonomy in patient care.
