Job summary

Trinity Health Of New England At Home is seeking a Home Health Registered Nurse (RN) to provide skilled nursing care in patients’ homes in Waterbury, CT. You will develop individualized care plans, administer medications, and guide families to support recovery and independence.

The role emphasizes top-of-licensure practice, autonomy, and meaningful 1:1 patient relationships, with a Monday–Friday schedule and weekend rotation. A $10,000 sign-on bonus is available for eligible candidates.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient progress.
• Educate patients and families to support recovery and independence.
• Coordinate with the care team to ensure seamless home health services.

Requirements

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse.
• Home care experience preferred but not required.
• Must have current Driver’s license and reliable transportation.
