Trinity Health Of New England At Home in the Enfield/Windsor region seeks a Home Health RN to deliver skilled nursing care at patients' homes. You will create and execute individualized care plans, administer medications, and educate families to support recovery and independence.

Qualifications

• Graduate of an approved nursing education program; BSN preferred.
• Active Connecticut RN license and BLS Certification.
• One year of professional acute care nursing experience.
• Home health experience preferred but not required.

Location:Enfield/Windsor Region

Status: Mon – Fri 8 am to 5 pm, weekend rotation every 4 weeks, 2

Requirements:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation
