Job summary

Trinity Health Of New England At Home in West Hartford is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes, create individualized care plans, administer medications, and educate patients and families to support recovery.

The role offers Mon-Fri 8 am–5 pm, with weekend rotation every 4 weeks and 2 holidays; features a $10,000 sign-on bonus, 1:1 patient relationships, and opportunities for professional growth in a nurse-led, supportive environment.

Qualifications

• CTRN licensure required; BSN preferred.
• One year of professional acute care nursing experience.
• Home care experience preferred but not required.
• Must have valid driver's license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Administer medications and monitor patient status.
• Educate patients and families to promote recovery and independence.
• Support coordination of care and documentation in patient records.

REQUIREMENTS:

• Graduate of an approved nursing education program, BSN preferred.
• Licensure as a Registered Nurse in the State of Connecticut.
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver's license and reliable transportation
