Job summary

HammondCare is seeking a Full-time Care Partner to join the Home Care Team at Miranda. You will deliver and develop tailored care plans for each client and oversee ongoing assessments to align care with needs and funding.

The role emphasizes care management, KPI tracking, and ensuring plans remain up-to-date while providing day-to-day services that meet service expectations. A driver’s licence and willingness to travel are required.

Qualifications

• Certificate IV in Aged Care and a minimum of 2 years relevant industry experience.
• Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
• Experienced in operating effectively in a multi-site environment.
• A current driver’s licence and willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request).
• Strong administrative and computer skills.

Responsibilities

• Deliver and develop tailored Care Plans for each client and oversee ongoing assessment of clients’ needs and preferences.
• Manage care of existing clients and client-related KPIs.
• Ensure care plans are up to date and aligned with client needs, approvals and funding.
• Provide professional day-to-day delivery of services in line with service expectations and ensure staff understand client needs and preferences.
