Job summary

Right at Home Hilton Head/Savannah is seeking a Care Coordinator to develop and maintain client care plans, communicating with clients, families, care partners and referral sources. You will ensure safe, meaningful care and mentor caregivers to support delivery.

The ideal candidate has two years of business experience, can work independently with good judgment, and is available for on-call duties. A valid driver’s license and insured vehicle are required.

Qualifications

• High school diploma or equivalent with two years of business experience.
• Able to work independently with sound judgment.
• Available for on-call duties outside normal hours.
• Valid driver's license and insured vehicle.

Responsibilities

• Develop and maintain client care plans and communicate with clients, families, and referral sources.
• Interfacing with clients and families to ensure safe, appropriate care.
• Mentor caregivers and support care delivery.
• Maintain a calm, positive demeanor and multitask in a busy office.
• Coordinate schedules for clients and caregivers.
• Create extraordinary client/family experiences and manage best-in-home-care plans.
