Requirements

• Home health experience is preferred but not required; training and mentorship provided
• Plan, prepare, and execute individualized programs to restore function and independence in the home setting
• Develop a plan of care and goals in consultation with physicians, families, and patients
• Timely and accurate documentation of visits
• Effective communication of discharge plans with families and team members

Qualifications

• Graduate of a Physical Therapist Program
• New Jersey Physical Therapist License or eligibility
• Evaluate patient needs and develop appropriate treatment plans
• Provide care based on physician care plans, agency policies, and professional standards
• Instruct family members, PTAs, aides, and caregivers in therapy techniques
• Certifications: CPR/First Aid
