Requirements:

• Home health experience is preferred but not required; training and mentorship provided for documentation and compliance
• Design and implement individualized therapy programs to restore function and independence at home
• Develop care plans/goals in consultation with physicians, families, and patients
• Timely and accurate documentation of visits
• Communicate discharge plans effectively with families and team members

Qualifications:

• Physical Therapist Program Graduate
• New Jersey Physical Therapist License or eligibility
• Assess patients' needs and develop appropriate treatment plans
• Provide care based on physician orders, agency policies, and professional standards
• Instruct family members, PTAs, aides, and caregivers in therapy techniques
• CPR/First Aid certifications required
