Job summary

Metro Physical & Aquatic Therapy is seeking a full-time Home Care Physical Therapist to provide in-home care across Central Suffolk County. Compensation ranges from USD 95,000 to USD 110,000 yearly, with a full benefits package and generous PTO.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Licensed PT in New York or eligible.
• Home health experience preferred.
• Ability to travel to patients' homes within territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $95,000.00-$110,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
