Job summary

Metro Physical & Aquatic Therapy in Eastern Suffolk County is seeking a motivated full-time Physical Therapist to provide in-home care across the area.

Competitive salary range of $95,000–$110,000 per year with a $3,000 sign-on bonus, full benefits, and generous PTO. All levels of experience are encouraged to apply, and mentorship is provided for new graduates.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in NY or eligible for licensure.
• Ability to travel to patients' homes within the assigned geographic territory.
• Home health experience preferred but not required; mentorship provided.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

Additional Information

The anticipated base salary range for this position is $95,000–$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
