Job summary

Metro Physical & Aquatic Therapy seeks a full-time Physical Therapist to provide in-home care across Eastern Suffolk County, including Medford and Patchogue. Base salary ranges from $95,000 to $110,000 per year, with a $3,000 sign-on bonus and full benefits.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in the state of New York or eligible for licensure.
• Home health experience is preferred but not required - we will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

Additional Information

The anticipated base salary range for this position is$95,000-$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

This position is eligible for a $3,000 sign-on bonus.Additional terms and conditions may apply.
