Job summary

Greenfield Rehabilitation Agency is expanding our home care team in Wisconsin. We are seeking a dynamic Physical Therapist for either full-time or part-time roles who can start delivering high-quality, patient-centered care from day one.

You’ll conduct comprehensive evaluations, develop individualized treatment plans, supervise PTAs as appropriate, and collaborate with an interdisciplinary team to achieve exceptional outcomes for our seniors in the community.

Qualifications

• Graduate of an accredited school with a bachelor’s degree in Physical Therapy.
• Wisconsin state licensure or eligibility.
• Minimum of one year practicing as a Physical Therapist.
• Strong clinical reasoning and teamwork.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans including OASIS paperwork.
• Deliver high-quality, evidence-based physical therapy services.
• Supervise PTAs as appropriate.
• Collaborate with an interdisciplinary team focused on exceptional outcomes.
• Build strong relationships and contribute to a positive therapy culture.
• Maintain accurate and compliant documentation.
