Job summary

Greenfield Rehabilitation Agency in Green Bay, Wisconsin is seeking an experienced Physical Therapist to join a comprehensive continuum-of-care senior campus. You will provide patient-centered therapy across multiple levels of care, collaborating with an interdisciplinary team to support transitions and maintain functional independence for seniors.

Graduate of an accredited PT program with Wisconsin licensure; supervision for PTA possible.

Qualifications

• Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
• Certification or license as required by Wisconsin or applicable state to practice as a Physical Therapist.
• Willingness to serve as a supervisor for a physical therapist assistant.

Responsibilities

• Provide meaningful, functional therapy that helps seniors maintain independence and quality of life.
