Qualifications

• Graduate of an accredited School of Physical Therapy.
• Licensed or eligible for licensure to practice in Connecticut.
• Experience with electronic medical charting (preferred).
• 3–5 years clinical experience.
• Flexibility with scheduling.

Responsibilities

• Develops a plan of care for each patient with input and agreement.
• Sets and revises goals based on patient progress and safety.
• Explains treatment goals, plans, techniques and outcomes to patient/family.
• Recommends therapeutic procedures and modalities.
• Maintains documentation standards and patient records.
• Addresses a home exercise program with each patient.
• Manages a caseload within productivity standards.
• Participates in continuing education and staff training.
