Metro Physical & Aquatic Therapy seeks a motivated in-home Physical Therapist (per diem) to provide high-quality care throughout the NY region. You will evaluate patients, develop individualized plans, and collaborate with families and physicians.

All experience levels are welcome; mentorship is provided for documentation and compliance. Flexible scheduling and remote options are offered, with 1099 and W-2 options to fit your work style.

Formación

• Licensed NY PT or eligible to practice in NY.
• Home health experience preferred but not required; mentor provided for documentation/compliance.

Metro is looking for a motivated and passionate per diem Physical Therapist to provide in-home care. Responsibilities:

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

Qualifications:
Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

Additional Information

The anticipated pay range for this position is $75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
