SWGA Homecare in Georgia seeks a field-based partnership and admissions specialist to drive referrals for non-medical home care services. You will leverage an existing network, present our offerings to hospital discharge planners, physician practices, and community leaders, and coordinate with our intake team for swift in-home assessments.

You will manage a referral pipeline, track conversions in the CRM, and expand relationships through health fairs and events to meet monthly admission targets.

Formación

• Must have active referral relationships in non-medical home care.
• Proven track record generating client referrals/admissions.
• Understanding non-medical vs skilled home health referrals.
• Self-motivated with outside-sales drive.
• Excellent presentation and follow-up skills.
• Comfortable with CRM-based pipeline tracking.

Responsabilidades

• Activate existing referral contacts across Southwest Georgia to generate client referrals from day one.
• Represent SWGA's non-medical services to referral sources and explain the difference from skilled home health.
• Respond quickly to inbound referrals and coordinate with Intake Coordinator and RN Supervisor for in-home assessments.
• Own referral pipeline from first contact through signed Service Agreement and track activity in CRM.
• Expand network through community events, health fairs, and provider networking in the service area.
• Meet or exceed monthly new-client admission targets and build early traction.
