Job summary

Interim HealthCare Inc. is seeking a Home Health Occupational Therapist (OT) to help patients regain independence at home. You will work within a home health team to implement personalized therapy plans and document progress toward goals.

The role requires CT OT licensure and at least 2 years of OT experience, preferably in home health. You’ll assess patients, guide exercises, and recommend adaptive equipment to improve safety and daily function.

Qualifications

• Graduate of an accredited Occupational Therapy Program and active OT license in CT.
• Minimum of 2 years of occupational therapy experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Collaborate with a home health team focused on the patient’s plan of care and goals.
• Assess patients, observe deficits, set therapy goals and document progress.
• Assist patients with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment to support independence and safety at home.
• Assess fall risks and educate patients on home safety strategies.
• Educate patient and family on plan of care, exercises, goals and self-care.

As a Home Health Occupational Therapist, here’s a big-picture view of what you’ll do:

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and SLP, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to offer added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

A few must-haves for Home Health Occupational Therapists:

• Graduate of an accredited Occupational Therapy Program and active OT license in CT
• Minimum of 2 years of occupational therapy experience, ideally in home healthcare
