Job summary

Interim HealthCare Inc. in Pasadena, CA, seeks a Home Health Occupational Therapist (OT) to provide patient-centered therapy in the home setting. The role emphasizes 1:1 patient care and integration with a home health team to achieve personalized goals.

Responsibilities include assessing patients, planning therapy, and educating families while adhering to home health regulations. A California OT license and CPR certification are required.

Qualifications

• Graduate from an accredited Occupational Therapy program and hold an active California OT license.
• Minimum of 1 year of occupational therapy experience, preferably in home health care.
• CPR certification and any other relevant certifications.
• Knowledge of California state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, resourcefulness and compassion.

Responsibilities

• Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
• Collaborate with a home health team (RN, LVN, CNA, HHA, PT/OT) on the plan of care.
• Assess patient, observe deficits, set therapy goals and document progress.
• Assist patient with exercises to improve fine motor skills and coordination.
• Suggest adaptive equipment to provide added support and improve safety at home.
• Assess fall risks and introduce strategies to improve home safety.
• Educate patient and family on plan of care, exercises, goals and self-care.

As a Home Health Occupational Therapist, here’s a big-picture view of what you’ll do:

• Provide occupational therapy to patients unable to perform daily tasks due to an illness or injury
• Work as part of a home health team which may include an RN, LVN, CNA, HHA, PT ,OT, focused on the patient’s plan of care and personal goals
• Assess patient, observe deficits, establish therapy goals and document progress
• Assist patient with exercises to improve fine motor skills and coordination
• Suggest adaptive equipment such as grab bars and shower chairs to provide added support
• Assess fall risks and introduce strategies to improve home safety
• Educate patient and family on plan of care, exercises, goals and self-care

A few must-haves for Home Health Occupational Therapists:

• Graduate of an accredited Occupational Therapy Program and active OT license in California
• Minimum of 1 years of occupational therapy experience, ideally in home healthcare
• CPR certification and any other certifications
• Knowledge of state and federal home health regulations
