Job summary

Advocate Health is seeking a full-time Occupational Therapist for Union, NC home health care. You will manage patient caseload, perform OT services, and coordinate with physicians and a multidisciplinary team across home settings.

The role requires OT licensure in the state, BLS for Healthcare Providers, and at least 1 year of experience preferred. Travel and documentation duties are included, with a sign-on bonus and referral incentives available.

Qualifications

• Graduation from an accredited OT program.
• Licensed to practice OT in the state.
• BLS HCP certification (AHA) required.

Responsibilities

• Manage an assigned patient caseload per plan of care and MD orders.
• Collect health data from home visits, phone contacts, and EMR.
• Evaluate functional abilities and develop the plan of care with the physician.
• Lead care coordination with a multidisciplinary team and supervise staff.
• Design and fit orthotic devices; train family and aides.
