RNS Private Duty Care in Kansas is seeking an LPN/RN Case Manager to provide clinical oversight for private-duty clients and supervise caregivers. You’ll split time between office, client visits, and home documentation in a true hybrid role.
You will complete reassessments, develop care plans, and coordinate with families and healthcare professionals. The role requires autonomy, strong clinical judgment, and excellent documentation skills, with weekend on-call rotation and mileage reimbursement.

RNS Private Duty Care is seeking an LPN or RN Case Manager to join our growing team and provide clinical oversight and support to our private-duty clients and caregivers.
This is a true hybrid position designed to provide flexibility while keeping you connected to the clients and caregivers you serve. You’ll spend one to two days per week in our office, with the remainder of your time split between client and caregiver visits in the field and working from home to complete documentation, care plans, coordination, and follow-up.
This position is ideal for someone who enjoys building relationships, working independently, and having a direct impact on the quality and consistency of care being provided in clients’ homes.
Complete 60-day client reassessment and supervisory visits and additional visits as needed
Complete initial and ongoing client assessments and identify changes in client needs
Develop, review, and update individualized care plans
Monitor client care and ensure caregivers are following the established plan of care
Conduct caregiver supervisory visits and provide coaching, education, and feedback
Train caregivers on client‑specific needs, care plans, safety, and appropriate care techniques
Communicate with clients, families, caregivers, physicians, and other healthcare professionals as needed
Identify changes in condition, safety concerns, or changes in care needs and communicate appropriately
Assist with caregiver orientation and client‑specific training
Review documentation and ensure clinical records are accurate and up to date
Work closely with scheduling and operations to address client and staffing needs
Participate in care conferences and assist with problem‑solving for complex client situations
Support quality assurance, compliance, and ongoing improvement of clinical services
Serve as a clinical resource and point of contact for caregivers and the office team
Participate in a rotating weekend on‑call schedule, approximately every 5th weekend, with a flat $200 stipend per on‑call weekend.
Other clinical and case management responsibilities as needed
Active, unrestricted LPN or RN license in Kansas
Valid driver’s license and reliable transportation- willingness to travel throughout the Kansas City metro area for client and caregiver visits (mileage reimbursement provided).
At least 1 years of LPN, RN experience (may substitute with 3 years home care/home health experience)
Strong assessment, clinical judgment, and documentation skills
Comfortable working independently and managing your own schedule
Comfortable communicating with clients, families, caregivers, and healthcare professionals
Ability to create and maintain individualized care plans
Ability to provide caregiver training, supervision, and feedback
Strong organizational and time-management skills
Comfortable using electronic documentation/scheduling systems
Ability to participate in the every-5th-weekend on-call rotation
