We are currently assisting one of our top home care agency partners with an experienced Home Care LPN who will see patients in in homes in the and around the Chelsea, MA area.

Position Details

• Full‑time, 40 hours/week, Monday‑Friday
• Occasional weekend rotation may be required
• Annual salary range: $72,000 - $87,000 (based on experience)
• Mileage reimbursement

Required Experience

Minimum of 1 year current/recent Home LPN experience with a Medicare certified agency.

Role and Responsibilities

• Provide direct patient care and patient advocacy assistance
• Coordinate care through communication with the multidisciplinary team and RN Case Manager
• Follow the nursing care plan established by the case manager based on nursing diagnoses, measurable goals, and specific interventions
• Report patient progress or changes to the primary nurse
