The ideal candidate has at least 1 year of home care intake experience, strong communication skills, and is highly organized with attention to detail.

As a Home Care Intake Specialist, you will play a critical role in processing patient referrals and ensuring a smooth transition into home care services. Role Overview:

As a Home Care Intake Specialist, you will be responsible for receiving and processing referrals from referral sources, contacting patients and families to explain services and gather required information, verifying patient insurance and determining eligibility, and educating patients and families on home care services. You will also work closely with referral sources, MLTC plans, Medicaid assistance resources, and internal teams to ensure each case moves forward properly.
