Job summary

Rockaway Home Care, serving patients through Michigan with a local Inwood Office, seeks an experienced Home Care Intake Specialist to manage referrals, insurance verification, and authorizations from referral to start of care.

You will coordinate with Medicaid/MLTC plans and care teams, review documentation, and ensure timely progress of each case while maintaining accurate records and professional communication with patients and partners.

Qualifications

• 1+ year of home care intake experience is required.
• Strong verbal and written communication skills are essential.
• Excellent organization and attention to detail are required.
• Comfortable using computers and learning new software and systems.

Responsibilities

• Receive, review, and process home care referrals from hospitals, physicians, facilities, community organizations, and other referral sources
• Contact patients and families to explain home care services and collect required information
• Verify patient insurance, eligibility, and coverage
• Review referral information and identify missing or additional documentation
• Coordinate with Medicaid, MLTC plans, insurance providers, and other community resources
• Submit referrals to appropriate plans or resources when enrollment or additional assistance is needed
• Follow up on insurance and service authorizations
• Determine which home care services have been approved
• Ensure required authorizations are obtained before services begin
• Communicate case updates and relevant information to the Coordination Department
• Maintain accurate and timely documentation in internal systems
• Follow up consistently on pending referrals, authorizations, and outstanding information
• Communicate professionally with patients, families, referral partners, insurance representatives, and internal teams
• Help ensure a smooth and timely transition from referral to start of care

Home Care Intake Specialist

Rockaway Home Care, a division of Foundation For the Elderly, is seeking an experienced Home Care Intake Specialist to join our team serving patients throughout Michigan in our Inwood Office!

This role is ideal for someone with hands‑on experience in home care intake, referrals, insurance verification, Medicaid, or care coordination who is highly organized and comfortable managing multiple cases from referral through authorization and start of care.

What You'll Do

As a Home Care Intake Specialist, you will help ensure patients receive the home care services they need by managing referrals, coordinating with insurance plans and referral sources, and keeping each case moving forward.

What We're Looking For

• 1+ year of home care intake experience required
• Knowledge of home care intake, referrals, authorizations, and insurance verification
• Understanding of Medicaid and MLTC processes
• Experience working with Medicare, Medicaid, MLTC, or managed care plans preferred
• Strong verbal and written communication skills
• Excellent organization and attention to detail
• Comfortable working with computers and learning new software and systems
• Strong follow-up skills and ability to manage multiple cases at once
• Ability to investigate information, identify issues, and determine appropriate next steps
• Self‑motivated and able to work independently
• Professional, dependable, and team‑oriented
• Strong customer service skills when communicating with patients and families
