Job summary

Addus HomeCare in Pleasant Hill, CA is seeking a Referral & Admissions Coordinator to orchestrate the referral and admissions process. You will interact with facility representatives, discharge planners, and community contacts to initiate care and ensure smooth transitions.

This full-time role requires at least one year of home care sales or related healthcare experience, strong documentation skills, and the ability to manage scheduling, paperwork, and communications with multiple stakeholders.

POSITION SUMMARY:

Responsible for the overall coordination of the referral and admissions process. Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care.

Hours:

Sunday-Thursday 11:30am-8:00pm PST

Essential Duties

• 1 year of Home Care Sales Required ***
• Provide explanation of the benefits to patients, families, and community contacts
• Assist with finding alternative services when patient is not a candidate for hospice
• Oversee scheduling of initial nursing assessment and admission
• Maintain admission packets and ensure that proper forms are available to admissions staff
• Knowledgeable in all Home Health and/or Hospice processes
• Assist community outreach representatives with marketing activities to promote relations with referral sources
• Assist with completion of admission paperwork as needed
• Adhere to work schedule and maintain good punctuality and attendance record
• Provide telephone and clerical support to other staff members as time allows
• Maintains accurate clinical records, keeping track of goals and progress of clients’
• Abides by all regulations, policies, procedures and standards
• Perform other duties as assigned

Position Requirements & Competencies

• Be eighteen (18) years of age or older
• At least one (1) year experience in a homecare, hospice and/or healthcare related field preferred
• Ability to complete necessary documentation appropriately, legibly and timely
• Ability to handle stressful and delicate situations
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
• Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
