Job summary

Qualifications
- Completion of a 120-hour Nursing Assistant course.
- High School Diploma or GED required.
- Must be at least 18 years old per regulatory requirements.
- 2–5 years of home care or hospice experience preferred.
Responsibilities
- Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility.
- Provide light housekeeping, meal preparation, and companionship.
- Support medication administration and simple procedures under RN guidance.
- Observe and report changes in patient condition or home environment to the RN Case Manager.
- Maintain accurate documentation and ensure compliance with care plans and regulatory standards.
- Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives.
- Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries.
