AccentCare is seeking a Field Supervisor for Personal Care Services in Illinois. The role involves supervising Home Care Aides, ensuring care plans are followed, and performing hands-on training with visits to client homes. Travel is required in Will and Cook Counties.

The ideal candidate has a high school diploma or GED, 2 years of home care/CNA experience, strong communication and problem-solving skills, and a valid driver’s license with reliable transportation.

Qualifications

• High School Diploma or GED required.
• 2 years of Home care or CNA experience.
• Strong communication, organizational, and problem-solving skills.
• Must have valid driver's license, reliable transportation, and auto insurance.
• Bilingual skills a plus, but not required.

Responsibilities

• Provides supervision and leadership to Home Care Aides.
• Evaluate the quality, effectiveness and of services that are needed for each of the clients served by HCA.
• Conduct unannounced quarterly home visits with clients that have Paid Family Caregivers to ensure the plan of care is being followed and HCA's are present during the hours scheduled.
• Coach, counsel, mentor and evaluate the performance of HCA's.
• Conduct home visits as necessary, to provide hands on training and assistance.
• Initiate and/or participate in clientstaffing discussion with Care Coordinator.
• Investigate allegations of fraud and theft reported by HCA.
• Documentation and follow up on incident reports.
