Responsibilities

• Schedules participant diagnostic appointments and medical consultations and enters appointments into the PACE system.
• Tracks appointments and prepares reports to monitor data.
• Coordinates with caregivers, transportation, and ALF staff to ensure attendance.
• Oversees documentation per department guidelines and regulatory standards.
• Provides coordination with other departments and supports the receptionist.
