We’ve combined core functions from two key roles to create a split position focused on patient care and quality improvement. The role offers flexible 8-hour workdays with a 7 days on, 7 days off schedule, providing significant time for family or personal pursuits. The position allows motivated nurses to shape their responsibilities further based on their strengths and interests.

Weekday Focus: Deliver high-level nursing care with proficiency in clinical skills.

Weekend Focus: Conduct assessments, review referrals, and liaise between Nascentia Health and healthcare providers. We can tailor the rest of your role to match your career goals.

Key Responsibilities:

• Weekend Duties: Assess patients, manage referrals, and visit facilities.
• Weekday Duties: Conduct patient visits and ensure continuity of care.

This role requires a flexible nurse capable of coordinating care plans and delivering direct patient care in the field.

Performance Responsibilities:

• Promote care continuity, develop care plans, and act as a liaison.
• Ensure quality assurance through documentation, resource coordination, and participation in quality improvement activities.
• Maintain accurate referral data and participate in team conferences.

Home Care Coordinator: Focuses on care planning, policy review, and collaboration with hospitals, SNFs, and other providers.

Field RN: Ensures safe, quality home health services, documents care accurately, and follows ethical standards.

Requirements:

• Current RN license or health-related degree in NY.
• Preferably 1+ year home health experience.
• Valid NY driver’s license and insurance.
• Excellent communication skills.

Physical & Safety Requirements: Ability to perform physical tasks, drive in all weather, and communicate effectively. Bloodborne pathogen training required.

Compensation & Benefits: Salary range $70,131–$81,275/year, with bonuses, mentoring, 401k, on-site gym, flexible schedule, health plans, tuition reimbursement, and more.
