Homewatch CareGivers of Lower Bucks County in Langhorne, PA, is seeking a Care Coordinator to ensure high-quality, coordinated in-home care. You will onboard new clients, match caregivers, maintain records, and communicate with families and healthcare professionals.

Ideal candidates are highly organized, customer-service oriented, with strong people skills and proficiency in Microsoft Office; this is a fast-paced role requiring meticulous attention to detail and compassionate service.

Qualifications

• High school diploma or equivalent; healthcare or admin experience preferred.
• Experience in home care or care coordination preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Coordinate the start of care for new clients and onboarding.
• Maintain accurate client and caregiver records in software systems.
• Communicate with clients, families, caregivers, and healthcare professionals about care needs and updates.
• Monitor client satisfaction through follow-up calls and QA check-ins.
• Assist with caregiver-client matching based on needs and qualifications.
• Track documentation and ensure policy and state regulation compliance.
• Support caregivers with questions and ongoing communication.
• Collaborate with office staff to ensure continuity of care and customer service.
• Assist with care plan updates and documentation as needed.

What You'll Do

• Coordinate the start of care for new clients and ensure a smooth onboarding process.
• Maintain accurate client and caregiver records in our software systems.
• Communicate with clients, families, caregivers, and healthcare professionals regarding care needs and updates.
• Monitor client satisfaction through routine follow-up calls and quality assurance check-ins.
• Assist with caregiver-client matching based on client needs and caregiver qualifications.
• Track required documentation and ensure compliance with company policies and state regulations.
• Support caregivers with questions, concerns, and ongoing communication.
• Collaborate with office staff to ensure continuity of care and excellent customer service.
• Assist with care plan updates and documentation as needed.

What We're Looking For

• Previous experience in home care, healthcare coordination, case management, or a related field preferred.
• Excellent communication and interpersonal skills.
• Strong organizational skills with exceptional attention to detail.
• Ability to prioritize multiple tasks in a fast-paced office environment.
• Proficiency with Microsoft Office and the ability to learn new software systems.
• Professional, compassionate, and customer-service oriented.
• High school diploma or equivalent required; additional healthcare or administrative experience preferred.
