Happier At Home Care, a Bucks County home care agency, seeks a Full/Part-time LPN or MA with home care experience. You will work both in the office and on the road in Central and Upper Bucks County, covering assigned client areas.

Responsibilities include coordinating client care, assisting families, documenting visits (WellSky), and ensuring high-quality care with ongoing communication to families, while traveling to client sites with mileage reimbursement.

Qualifications

• Meeting with clients to go over care plan and any changes in care
• Random and Announced visits, minimum once a week
• Hospitalization visits and Post Hospitalization visits
• Communication with the Family

Job Description

Happier At Home Care is a non-medical home care agency located in Bucks County. We are looking for a Full and part time LPN's or MA's with experience in the home care field. Our clients are located throughout Bucks, and Montgomery County. (You will have an assigned area).

Care Coordinator responsibilities

Care Coordinator responsibilities will be to ensure that both clients, families and caregivers are assisting the client with the highest quality of care available. The Care Coordinator role has been developed to ensure that all the clients’ needs are being met through our services and that they obtain outside services as needed in the community. The dedicated Care Coordinator will provide the following services.
