Qualifications

• Associate degree in Healthcare Administration, Business Administration, Nursing, Human Services, or related field preferred.
• Minimum of 2 years of experience in home health care, healthcare coordination, scheduling, staffing, or healthcare operations preferred.
• Must have a dependable automobile and a clean driving record.
• Experience working with caregiver management systems and electronic health record platforms preferred.

Core hours of 8:30am– 4:30pm; Essential: Some weekend and extended weekday hours may be required even in the event of inclement weather; Daily Job Site: Homebase/ Mobile.

FLSA STATUS: Exempt

CLASIFICATION: Full Time

COMPENSATION: $40K/year

FIELD WORK & TRAVEL: Position requires 35% travel

The position of the Home Care Coordinator is responsible for coordinating client care services, caregiver scheduling, compliance monitoring, referral relationship management, and operational support to ensure exceptional service delivery. This role serves as a vital link between clients, caregivers, referral sources, and leadership, helping to drive organizational growth while maintaining regulatory compliance and quality standards. The Home Health Care Coordinator works collaboratively with the Director of Nursing, Recruiter, CEO, caregivers, clients, and referral partners to support EPIC's mission of delivering outstanding home healthcare services. Ensures that EPIC’s existing database structure is utilized by CAREGIVER s as required by regulation. Overall, ensures EPIC’s in-home care services operate smoothly.

QUALIFICATIONS &REQUIREMENTS:

• Associate degree in Healthcare Administration, Business Administration, Nursing, Human Services, or a related field preferred.
• Equivalent combination of education and experience may be considered.
• Minimum of 2 years of experience in home health care, healthcare coordination, scheduling, staffing, or healthcare operations preferred.
• Experience working with caregiver management systems and electronic health record platforms preferred.
• Knowledge of home care regulations and compliance requirements preferred.
• Must have a dependable automobile and a clean driving record.
