Job summary

Clearbrook is seeking a Home-Based Services Coordinator in Arlington Heights, IL to supervise case managers and support program operations. The role reports to the Director and works with coordinators across programs, maintaining vital contacts with families and providers.

Responsibilities include upholding confidentiality, facilitating staff meetings and applying the PHI coaching approach with the team; full-time hours, salary in the listed range.

Qualifications

• Bachelor's degree in a social service related field.
• At least 3 years of direct experience with individuals with intellectual disabilities.
• Valid IL driver's license.
• Minimum of 2 years supervisory experience.
• At least 2 years of case management experience.

Responsibilities

• The Home-Based Services Coordinator supervises case managers and reports to the Director.
• Manages program operations in the absence of the Director and collaborates with other coordinators.
• Maintains contacts with families, guardians, and external service providers.
• Ensures confidentiality per Clearbrook policy and facilitates staff meetings.
• Implements the PHI coaching approach with the team.

Clearbrook, a leading non-profit social services agency, is seeking a skilled and compassionate professional to join our team as a Coordinator for our Home-Based Services.

The Home-Based Services Coordinator supervises our case managers and reports directly to the Director, managing program operations in their absence. In this role, you will collaborate with fellow coordinators across Clearbrook programs and maintain vital contacts with families, guardians, and external service providers. You must ensure strict compliance with Clearbrook’s confidentiality policy, facilitate staff meetings for timely communication, and effectively implement the PHI coaching approach with your team.

Qualifications

• Bachelor's degree in a social service related field


• At least 3 year of experience working directly with individuals with intellectual disabilities


• Valid IL driver's license


• Minimum of 2 years supervisory experience is required


• At least 2 years of case management experience
