Job summary

Lutheran Life Communities in Arlington Heights, IL is hiring a Social Services Coordinator to support resident well-being and coordinate psychosocial services for residents and families.

The role requires social services experience in a senior living setting, discharge planning experience, and PCC proficiency. Full-time hours are 9:00 AM–5:00 PM with rotating Sundays, and a comprehensive benefits package is provided.

Qualifications

• Minimum 2 years of social services experience in a senior living setting.
• 2+ years of discharge planning experience in senior living.
• 1+ year of experience using PointClickCare (PCC).

Responsibilities

• Complete social service assessments to identify resident needs and preferences.
• Coordinate discharge planning and connect residents with outside psychiatric and psychosocial services.
• Advocate for residents and support families through care transitions.
• Facilitate family involvement in care planning meetings.
• Connect residents with community resources and encourage participation in facility programming.
• Document social service plans and progress notes in PointClickCare.
• Collaborate with nursing, therapy, and the rest of the care team on resident care plans.

$58,000–$64,000/year, based on experience. Full-time, 9:00 AM–5:00 PM, with rotating Sundays. Daily pay access and 24/7 chaplain support included. Join our skilled nursing team in Arlington Heights, IL. We have two openings on this team.

The Lutheran Home Skilled Nursing Community is hiring Social Services Coordinators to support resident well-being and coordinate psychosocial services for our residents and families.

What You'll Do

• Complete social service assessments to identify each resident's needs and preferences
• Coordinate discharge planning and connect residents with outside psychiatric and psychosocial services

What You'll Need

Required:

Preferred

• Experience working with Medicare/Medicaid resident populations

Schedule
