Job summary

True Care is seeking a Coordination Team Lead in Brooklyn, NY. This role provides day-to-day oversight, coaching, and performance management for Scheduling Coordinators, ensuring policy adherence and high-quality patient care.

You will coordinate with multiple departments, handle contract-related tasks, and monitor patient satisfaction while maintaining clear communications with aides and families.

Qualifications

• Fluent in English and Spanish to liaise with patients and families.
• Strong communication skills with employees, patients, and affiliates.
• Proficient in Microsoft Office Suite; experience with HHA Exchange preferred.

Responsibilities

• Oversee day-to-day operations of the Coordination team.
• Coach and train team members and conduct performance reviews.
• Ensure adherence to policies, procedures, and staffing protocols.
• Manage contracts, scheduling, and interdepartmental communication.
